Starting a placement at Madigan? Here's everything to have ready.
Required identification
- Birth Certificate
- Driver's License / Identification Card
- Social Security Card
Immunization records
- Tdap within the last 10 years
- 3-dose Hepatitis B / A series or positive titer
- 2-dose Varicella or positive titer
- Current flu shot
- TB test within the last 12 months
Required forms
- Form 1: Personnel Security Investigation Portal (PSIP) Information Sheet
- Form 2: OF-306 — Declaration for Federal Employment
